How to Make a Claim
If you are a member of Little Athletics Australia State / Territory Association and you have suffered an injury whilst participating in an official Little Athletics Australia activity (including training, competitions, races, or other sanctioned activities) please follow the below steps to lodge a Personal Accident claim.
Please refer to the policy wording for policy terms, conditions and exclusions.
Step 1: Download a Little Athletics Australia Personal Accident claim form
Step 2: Complete each section of the claim form. Please be advised that incomplete claim forms may cause a delay in processing your claim.
Step 3: Please return your Personal Accident claim form and supporting documentation to your State Association to sign.
Step 4: Your State / Territory Association will sign your claim form to confirm your membership, and will forward your claim form onto Corporate Services Network who will assess your claim.
Step 5: Corporate Services Network will confirm receipt of your claim and advise whether they require further information.

Once your claim is registered, additional receipts can be submitted to Corporate Services Network (GPO Box 4276, Sydney NSW 2001) or send via email to email@example.com and quote the claim number/reference you have been given.
The Health Insurance Act (Cth) 1973 does not permit the insurer to contribute to any charges covered, or partially covered by Medicare.

In addition, there will be no refund in respect of any expense to which the National Health Act (Cth) 1953 or any of the regulations made there under apply.

In the event of a property claim arising, immediate notification should be given to V-Insurance Group who will help you through the claims process. In the event of large losses, an assessor may be appointed to review and assist.
V-Insurance will provide you with a claim form to be completed and returned to your State Association.

CYBER LIABILITY & PRIVACY PROTECTION
As soon as you become aware of a potential Cyber Liability claim or a breach of Privacy, you must notify the insurer Dual. Dual have an incident hotline in place to help you manage matters. This incident hotline is handled by the International Service provider, Charles Taylor and will help you with finding a resolution to your matter.
